Objetivo: A proteína HER2 identifica um subtipo mais agressivo de câncer de mama. Para esse tipo de tumor, atualmente existem tratamentos específicos (trastuzumabe e lapatinibe), que, se usados na fase inicial da doença, podem proporcionar uma maior chance de cura e sobrevivência para os pacientes (o estudo HERA mostrou que o uso de Herceptin após uma quimioterapia padrão em tumores adjuvantes reduziu em 46% o risco de recidiva do tumor). O objetivo do estudo é avaliar o desempenho dos testes diagnósticos dessa proteína para pacientes Brasileiros com câncer de mama. Métodos: Foi utilizado um estudo diário com 220 oncologistas clínicos Brasileiros; ao final do estudo, um total de 3.994 pacientes com câncer de mama foi acompanhado. Resultados: Cinquenta e oito por cento dos pacientes encontrados estavam no mercado público e 42% estavam no mercado privado. Além disso, 64% dos pacientes tinham tumores em estágio inicial (adjuvantes ou neoadjuvantes) e 36% tinham tumores metastáticos. O teste HER2 é realizado em 80% dos pacientes no mercado privado, com apenas 36% dos pacientes do mercado público sendo testados. A taxa de desempenho do teste tem pouca correlação com a fase da doença (valor de P = 0,81); no entanto, há alguma correlação do desempenho desse teste com a idade do paciente (valor de P = 0,3). Médicos de vários estados Brasileiros apresentam comportamentos diferentes ao realizar esse exame (valor de P = 6^162). Conclusão: Verificamos que poucos pacientes do mercado público (36%) realizam esse teste em comparação com o mercado privado (80%). Também verificamos grandes diferenças no desempenho do exame nas diferentes regiões do país. Aumentar a taxa de realização desse exame aumentaria as chances de sobrevivência e cura dos pacientes, uma vez que permite o uso de tratamentos específicos para tumores HER2 positivos.

Abstract
Objetive: Protein HER2 identifies a more aggressive subtype of breast cancer. For this type of tumor, currently there are specific treatments (trastuzumab and lapatinib), which, if used in early phase of the disease can render a higher chance of cure and survival to the patients (the HERA study showed that the use of Herceptin after a standard chemotherapy in adjuvant tumors has reduced by 46% the risk of a tumor to return). The study Objetive is to assess the performance of the diagnostic tests of this protein for Brazilian patients with breast cancer. Methods: A diary study with 220 Brazilian clinical oncologists was used; in the end of the study, a total of 3994 patients with breast cancer were followed up. Results: Fifty-eight percent of the patients found were in the public market, and 42% were from the private market. We also have that 64% of the patients have early tumors (adjuvant or neoadjuvant), and 36% have metastatic tumors. The HER2 test is performed in 80% of the patients in the private market, with only 36% of the patients of the public market being tested. The test performance rate has little correlation with the disease phase (P value = 0.81); however, there is some correlation of the performance of this test with the patient's age (P value = 0.3). Physicians from several Brazilian states have different behavior while performing this exam (P value = 6^162) Conclusion: We verified that few patients of the public market (36%) perform this test comparing with the private market (80%). We also verified great differences in the performance of it in the different regions of the country. An increased performance rate of this exam would increase the chances of survival and cure for the patients, since it allows the use of specific treatments for positive HER2 tumors.
