Resumo
Objetivos: Quase 90% de todas as diálises realizadas no Brasil são feitas por hemodiálise (HD). Estimativas recentes mostram que o custo direto anual de fornecer HD pelo governo Brasileiro é de US$7.980 por paciente. Também foi estimado que a doença renal em estágio terminal (ESRD) está crescendo anualmente a uma taxa de 10,1%, mas aproximadamente 25% dos pacientes não são tratados. O objetivo do presente estudo foi estimar o impacto do tratamento de uma população crescente e não atendida de ESRD nas despesas governamentais. Métodos: Foi utilizado um modelo de impacto orçamentário baseado em Excel para estimar o impacto cumulativo de 5 anos de um aumento no tratamento de pacientes com ESRD usando HD. O ano de 2006 foi o ano base e o modelo projetou custos até 2011. No ano base, havia 62.300 pacientes em HD. O modelo assumiu uma taxa de crescimento anual de ESRD de 10,1%. Os custos diretos de HD foram mantidos constantes em US$7.980 ao longo da projeção de cinco anos. Os custos são do ponto de vista do governo. Foi assumido que 25% dos pacientes com ESRD estão atualmente sem tratamento. Resultados: No ano base, o custo direto total de HD é de US$358 milhões. Se não houvesse crescimento em ESRD, os custos cumulativos de tratamento em 5 anos seriam de US$2,15 bilhões. No entanto, com um crescimento de 10,1% em ESRD, os custos cumulativos de 5 anos seriam de US$2,77 bilhões, um aumento líquido de US$621 milhões. Além disso, se o tratamento com HD for fornecido a todos os pacientes não tratados com ESRD (20.767 pacientes adicionais no ano base), o orçamento cumulativo de 5 anos aumenta para US$3,7 bilhões. Conclusão: O tratamento de pacientes com ESRD com HD é substancial. Se o governo do Brasil tiver como alvo tratar todos os pacientes com HD, os custos da diálise disparariam. Para manter esses pacientes vivos com boa qualidade de vida, o governo deve considerar mais de perto outros modos de diálise que sejam clinicamente eficazes e econômicos.

Abstract
Objetives: Nearly 90% of all dialysis performed in Brazil is done by hemodialysis (HD). A recent estimates shows that the annual direct cost of providing HD by the Brazilian government is $7,980 per patient. It is also been estimated that end-stage renal disease (ESRD) is growing annually at 10.1%, yet approximately 25% of patients are not treated. The Objetive of the present study was to estimate the impact of treating a growing and underserved ESRD population on government expenditures. Methods: An Excel-based budget impact model (BIM) was used to estimate the cumulative 5-year impact of an increase in the treatment of ESRD patients using HD. The year 2006 was the baseline and the model projected costs out to 2011. At baseline, there were 62,300 HD patients. The model assumed an annual ESRD growth rate of 10.1%. The direct costs of HD were held constant at $7,980 over the five-year projection. Costs are from the government point of view. It was assumed that 25% of ESRD patients are currently untreated. Results: At baseline, the total direct cost of HD is $358 million. If there were no growth in ESRD, the 5-year cumulative treatment costs would be $2.15 billion. However with a 10.1% ESRD growth, the cumulative 5-year costs would be $2.77 billion, a net increase of $621 million. In addition, if HD treatment is provided to all untreated patients with ESRD (20,767 additional patients at baseline), the cumulative 5-year budget increases to $3.7 billion. Conclusion: Treatment of ESRD patients with HD is substantial. If the government of Brazil targets to treat all patients with HD, the costs of dialysis would skyrocket. To keep these patients alive with a good quality of life, the government should look more closely at other modes of dialysis that are both clinically effective and cost saving.
