Introdução: O tromboembolismo venoso agudo (TEV) está associado a alta morbidade e mortalidade. Há um grande volume de CPGs publicados internacionalmente para TEV que causa um dilema para a saúde na Arábia Saudita. Portanto, o objetivo deste artigo é descrever a adaptação sistemática das diretrizes internacionais de TEV no contexto dos ambientes de saúde sauditas Método: foi usada a estrutura 'King Saud University (KSU)-Modified-ADAPTE'. Isso incluiu: as fases de configuração, adaptação e finalização ao longo de 18 meses (2018-2020). Uma equipe multidisciplinar liderou o processo de adaptação, que incluiu uma avaliação externa do conteúdo clínico e da metodologia. Resultados: O grupo adaptou dez categorias principais de recomendações de três CPGs de origem (NICE, SIGN e ACEP). O CPG adaptado incluiu recomendações abordando três seções: (i) triagem de TEV em pessoas ambulatoriais, de emergência, médicas, cirúrgicas e grávidas, reavaliação do risco de TEV e sangramento e investigações clínicas e laboratoriais; (ii) profilaxia de TEV em pacientes ambulatoriais, de emergência, médicos, cirúrgicos e obstétricos, além de trombose relacionada a viagens; e (iii) gerenciamento de TEV, incluindo TEV agudo, embolia pulmonar, TVP de membros inferiores, trombólise e terapia farmacomecânica, tromboflebite superficial, TVP de extremidade superior, trombose da veia esplâncnica, TEV incidental, gerenciamento adicional de TEV e efeitos adversos da profilaxia e tratamento de TEV. Conclusão: O CPG adaptado finalizado fornece recomendações aplicáveis baseadas em evidências para o gerenciamento de adultos com TEV na Arábia Saudita. Também demonstrou uma história de sucesso de colaboração entre os profissionais clínicos nacionais e os especialistas metodológicos internacionais para adaptação de CPGs no contexto de ambientes de saúde sauditas.

Abstract
Introduction: Acute venous thromboembolism (VTE) is associated with high morbidity and mortality. There is a large volume of internationally published CPGs for VTE that causes a dilemma for healthcare in Saudi Arabia. Hence, the aim of this article is to describe the systematic adaptation of VTE international guidelines in the context of Saudi healthcare settings Method: the ‘King Saud University (KSU)-Modified-ADAPTE’ framework was used. This included: the setup, adaptation, and finalization phases over 18 months (2018-2020). A multidisciplinary team led the adaptation process, which included an external assessment of the clinical content and methodology. Results: The group adapted ten main categories of recommendations from three Source CPGs (NICE, SIGN, and ACEP). The adapted CPG included recommendations addressing three sections: (i) VTE screening in the ambulatory, emergency, medical, surgical, and pregnant people, reassessment of risk of VTE and bleeding, and clinical and laboratory investigations; (ii) VTE prophylaxis in ambulatory, emergency, medical, surgical, and obstetric patients in addition to travel-related thrombosis; and (iii) VTE management including acute VTE, pulmonary embolism, lower limb DVT, thrombolysis and pharmaco-mechanical therapy, superficial thrombophlebitis, upper extremity DVT, splanchnic vein thrombosis, incidental VTE, further management of VTE, and adverse effects of VTE prophylaxis and treatment. Conclusion: The finalized adapted CPG provides applicable evidence-based recommendations for the management of adults with VTE in Saudi Arabia. It also demonstrated a success story of collaboration between the National clinical professionals and the International methodological experts for adaptation of CPGs in the context of Saudi healthcare settings.
