Introdução: A artrite reumatoide (AR) é uma doença crônica, inflamatória e autoimune que afeta as articulações. bDMARDs e tsDMARDs ajudaram a expandir o arsenal terapêutico para AR e mudaram significativamente a forma como a doença é manejada. No entanto, ainda existem necessidades não atendidas para pacientes que falham ao tratamento. O objetivo deste estudo foi avaliar a efetividade desses medicamentos em cenários do mundo real por meio de um estudo de coorte. Métodos: Este estudo comparou a efetividade entre inibidores de Janus quinase (iJAK [tsDMARDs]), anti-TNF e não anti-TNF (bDMARDs) por meio da persistência ao tratamento em terapias de 1ª e 2ª linha. O acesso a esses medicamentos requer um processo de aprovação administrativa no Sistema Público de Saúde brasileiro (BPHS). Esta coorte incluiu pacientes com AR que solicitaram seu primeiro medicamento entre junho de 2018 e setembro de 2022. Foi conduzido em farmácias do BPHS (Estado de Minas Gerais) e construído a partir de registros de dispensação de medicamentos, dados clínicos e sociodemográficos. Análise de variância e curvas de Kaplan-Meier foram utilizadas para comparar os tempos médios de persistência e a persistência em 18 meses de acompanhamento. Resultados Entre 763 pacientes, 85,1% eram do sexo feminino, 33,0% brancos, com idade média de 55,4 anos e 6,5 anos de diagnóstico. Inicialmente, 68,2% usaram anti-TNF (adalimumabe, etanercepte, infliximabe, certolizumabe pegol e golimumabe), 7,2% não anti-TNF (abatacepte e tocilizumabe) e 24,6% agentes iJAK (tofacitinibe, baricitinibe, upadacitinibe). O grupo não anti-TNF apresentou menor persistência ao tratamento do que os outros grupos (p0.05). Os grupos de baricitinibe e tocilizumabe mostraram maior persistência. Na 2ª linha, foi observada uma diminuição geral na persistência ao tratamento. Conclusões iJAK apresentou a melhor persistência na 1ª e 2ª linha, e não houve diferença entre os grupos de medicamentos na 2ª linha. No entanto, a taxa de descontinuação e troca de alguns medicamentos sugere falha sistemática do tratamento. Além disso, os resultados sugerem que os pacientes respondem menos à 2ª linha. Esses resultados são úteis para auxiliar futuras reavaliações e decisões de gestores de saúde.

Abstract
lntroduction: Rheumatoid arthritis (RA) is a chronic, inflammatory, autoimmune disease that affects the joints. bDMARDs and tsDMARDs have helped expand the therapeutic armamentarium for RA and significantly changed the way the disease is managed. However, there are still unmet needs for patients who are treatment failures. The aim of this study was to evatuate the effectiveness of these grugs in real-world settings through a cohort study. Methods This study compared the effectiveness between Janus kinase inhibitors (iJAK [tsDMARDs]), anti-TNF and non-anti-TNF (bDMARDs) through treatment persistence in 1st and 2nd line therapy. Access to these drugs requires an administrative approval process in the Brazilian Public Health System (BPHS). This cohort included RA patients who requested their first drug between June 2018 and September 2022. It was conducted in pharmacys of the BPHS (Minas Gerais State) and was constructed from drug dispensing records, clinical and sociodemographic data. Analysis of variance and Kaplan-Meier curves were used to compare the mean persistence times and the persistence at 18 months of follow-up. Results Among 763 patients, 85.1% were tamale, 33.0% white, with a mean age 55.4 years and 6.5 years of diagnosis. Initially 68.2% used anti-TNF (adalimumab, etanercept, infliximab, certolizumab pegol and golimumab), 7.2% non-anti-TNF (abatacept and tocilizumab) and 24.6% iJAK agents (tofacitinib, baricitinib, upadacitinib). Non-anti-TNF group had lower treatment persistence than the other groups (p0.05). Baricitinib and tocilizumb groups showed higher persistence. In the 2nd line, a general decrease in the treatment persistence was observed. Conclusions iJAK showed the best persistence in 1st and 2nd line, and there was no difference between drug groups in 2nd line. However, the rate of discontinuation and switching of some drugs suggests systematic treatment failure. Furthermore, the results suggest that patients respond less to 2nd line. These results are useful to help the future reassessment and decisions of health managers.
