Resumo
Introdução: Como etapa do processo de incorporação de tecnologias, a Agência Nacional de Saúde Suplementar (ANS) elabora relatórios de avaliação crítica (RACs). A análise desses relatórios pode auxiliar na compreensão das demandas na saúde suplementar. O objetivo deste trabalho é caracterizar as tecnologias submetidas à ANS e incorporadas, quanto ao tipo de intervenção, grau de certeza das evidências e aspectos econômicos. Métodos: Foi realizado um estudo descritivo no qual foram analisados dados dos RACs das propostas de atualização do Rol incorporadas entre outubro de 2021 e dezembro de 2023. Os dados extraídos incluíram: tipo de intervenção, classificação da doença/condição, nível de certeza da evidência para o desfecho principal, razão de custo-efetividade incremental (ICER) e impacto orçamentário incremental (IBI) em cinco anos. Para avaliação do ICER, considerou-se a porcentagem de tecnologias em que o custo excedeu três vezes o Produto Interno Bruto (PIB) brasileiro de 2023 (USD 31.103,4557). Os dados foram apresentados por meio de estatística descritiva. Resultados: Das 50 tecnologias incorporadas, 96% eram terapêuticas e 4% preventivas/profiláticas. A maioria das propostas foi para condições oncológicas (68%). A certeza das evidências foi moderada/alta em 52% das incorporações e baixa/muito baixa em 48%. O impacto orçamentário incremental médio foi de USD 32.001.783,74. Em 24% das incorporações, o impacto foi negativo, sendo que 75% destas apresentaram baixa/muito baixa certeza das evidências. Foi realizada análise de minimização de custos para 15 propostas; para as demais, a razão de custo-efetividade incremental foi superior a três vezes o PIB em 54,3% das incorporações. Conclusões: Uma parcela relevante das tecnologias incorporadas pela ANS apresentou baixa/muito baixa certeza das evidências. Os custos excederam três vezes o PIB brasileiro em mais da metade das tecnologias em que o ICER foi calculado. Os resultados destacam a necessidade de decisões baseadas em evidências robustas e custo-efetividade, a fim de garantir a sustentabilidade do sistema de saúde suplementar.

The analysis of the RACs can help in understanding the demands in supplementary health. The aim of this work is to characterize the technologies submitted to ANS and incorporated, regarding the type of intervention, certainty of evidence, and economic aspects. Methods A descriptive study was conducted in which the data from the RACs of the proposals for updating the Rol incorporated between October 2021 and December 2023 were analyzed. The extracted data included: type of intervention, classification of the disease/condition, levei of certainty of the evidence for the main outcome, incremental cost-effectiveness ratio (ICER), and incremental budget impact (IBI} over five years. For the ICER evaluation, the percentage of technologies where the cost exceeded three times the Brazilian Gross Domestic Product (GDP) of 2023 (USD 31,103.4557) was considered. The data were presented through descriptive statistics. Results Of the 50 incorporated technologies, 96% were therapeutic and 4% were preventive/prophylactic. The majority of the proposals were for oncological conditions (68%). The certainty of the evidence was moderate/high in 52% of the incorporations and low/very low in 48%. The average incremental budget impact (IBI} was USD 32,001,783.74. ln 24% of the incorporations, the IBI was negative, with 75% of these having low/very low evidence certainty. A cost-minimization analysis was conducted for 15 proposals; for the others, the incremental cost-effectiveness ratio (ICER) was greater than three times the GDP in 54.3% of the incorporations. Conclusions A relevant percentage of the technologies incorporated by ANS had low/very low certainty of evidence. Costs exceeded three times the Brazilian GDP in more than half of the technologies where the ICER was calculated. The results highlight the need for decisions based on robust evidence and cost-effectiveness to ensure the sustainability of the supplementary health system.
