Introdução: O Ambulatório de Anticoagulação (AMBAC) do Hospital Universitário Professor Edgard Santos (HUPES) atende pacientes em uso de varfarina desde 2005, com diagnóstico de Fibrilação Atrial (FA), Acidente Vascular Cerebral Isquêmico (AVCi), trombofilias e Prótese Valvar Mecânica (PVM). Entretanto, durante a pandemia de COVID-19 fez-se necessário a alteração do formato de atendimento aos pacientes, considerando a emergência de saúde pública e a obrigatoriedade do isolamento social. Em julho de 2022 o Conselho Federal de Farmácia normatizou através da Resolução nº 727/2022 a consulta via telefarmácia, desta forma, tona-se imperativo analisar os resultados relacionados ao seguimento farmacoterapêutico dos pacientes atendidos por teleconsulta. Objetivos: Descrever o perfil de pacientes em uso de varfarina assistidos por intermédio da teleconsulta farmacêutica no AMBAC. Métodos: Trata-se de um estudo descritivo, de corte transversal e retrospectivo. Os dados foram obtidos através da revisão de prontuário dos pacientes no AGHU, assistidos mensalmente por teleconsulta através de chamadas de vídeo, mediante o uso de dispositivos móveis. O período de estudo foi de julho a outubro de 2022. Os dados foram analisados no Microsoft Excel®. A faixa terapêutica do Relação Normatizada Internacional (RNI) estabelecida na literatura é de 2,0 a 3,0 para portadores de FA e outras patologias e de 2,5 a 3,5 para pacientes com PVM. Resultados e Discussão: O total de pacientes atendidos no referido período foi de 55 pacientes, sendo 37 pacientes do sexo feminino. A média de idade foi de 59 e 56 anos para mulheres e homens respectivamente, já a mediana foi de 54 anos para ambos os grupos. A idade máxima foi 86 anos e mínima foi de 32 anos. Quanto a localidade, 69% (N=38) são moradores de Salvador e Região Metropolitana e 31% (N=17) interior da Bahia. A patologia com maior prevalência foi PVM com 62% (N=34) dos pacientes, seguido de FA com 33% (N=18) e os demais 5% (N=03) possuem outras patologias. Com relação ao controle do RNI, temos um total de 69% (N=38) com RNI mantido dentro da faixa terapêutica, sendo 58% (N=22) com PVM, 39% (N=15) com FA e 3% (N=01) portador de trombose, para os pacientes com controle do RNI a dose média semanal de varfarina foi de 35,9 mg, sendo de aproximadamente 29,4 mg para FA e de 39,6 mg para PVM. Os extremos de dose semanal registrada para o controle de RNI em pacientes com PVM foram de 72,5 mg e de 15 mg, respectivamente, os dados obtidos são semelhantes aos encontrados na literatura para pacientes em acompanhamento presencial em uso de varfarina. Conclusão: O controle do RNI nos pacientes em uso de varfarina, através da teleconsulta farmacêutica demonstrou ser uma estratégia efetiva e segura para os pacientes, entretanto, novos estudos são necessários para avaliação a médio e longo prazo desta prática.

Introduction: The Anticoagulation Outpatient Clinic (AMBAC) at the Professor Edgard Santos University Hospital (HUPES) has been attending patients on warfarin since 2005, diagnosed with Atrial Fibrillation (AF), Ischemic Stroke (IS), thrombophilias, and Mechanical Heart Valve (MHV). However, during the COVID-19 pandemic, it became necessary to change the patient care format considering the public health emergency and mandatory social isolation. In July 2022, the Federal Pharmacy Council regulated telepharmacy consultations through Resolution No. 727/2022, making it imperative to analyze the outcomes related to pharmacotherapeutic follow-up of patients attended through teleconsultation. Objectives: To describe the profile of patients on warfarin therapy assisted through pharmaceutical teleconsultation at AMBAC. Methods: This is a descriptive, cross-sectional, retrospective study. Data were obtained through patient record review in the AGHU system, where patients were monthly assisted via video calls using mobile devices. The study period was from July to October 2022. Data were analyzed using Microsoft Excel®. The therapeutic range of International Normalized Ratio (INR) established in the literature is 2.0 to 3.0 for patients with AF and other pathologies and 2.5 to 3.5 for patients with MHV. Results and Discussion: A total of 55 patients were attended during the study period, with 37 female patients. The mean age was 59 years for women and 56 years for men, with a median of 54 years for both groups. The maximum age was 86 years and the minimum was 32 years. Regarding location, 69% (N=38) were residents of Salvador and the Metropolitan Region, and 31% (N=17) were from the interior of Bahia. The most prevalent condition was MHV in 62% (N=34) of patients, followed by AF in 33% (N=18), and the remaining 5% (N=03) had other pathologies. Regarding INR control, 69% (N=38) of patients maintained their INR within the therapeutic range, with 58% (N=22) having MHV, 39% (N=15) AF, and 3% (N=01) with thrombosis. For patients with controlled INR, the average weekly warfarin dose was 35.9 mg, approximately 29.4 mg for AF, and 39.6 mg for MHV. The extremes of weekly dose recorded for INR control in patients with MHV were 72.5 mg and 15 mg, respectively. The data obtained are similar to those found in the literature for patients attending in-person follow-ups on warfarin. Conclusion: INR control in patients on warfarin therapy through pharmaceutical teleconsultation has proven to be an effective and safe strategy for patients. However, further studies are necessary to evaluate the medium and long-term outcomes of this practice.
