Resumo
Introdução: O RARAS (Rede Nacional de Doenças RARAS) é um estudo desenvolvido nos hospitais de referência no manejo de pacientes com doenças raras no Sistema Único de Saúde (SUS). Objetivo: Quantificar e descrever os custos associados a jornada assistencial do paciente com Polineuropatia Amiloidótica Familiar (PAF) como parte do projeto JAV-RARAS, que é um componente da Rede Nacional de Doenças Raras (RARAS) no Brasil aplicando o Time-driven activity-based costing (TDABC). Material e Método: O estudo do custeio baseado em atividades e tempo (TDABC) consiste nos dados coletados de diagnóstico, tratamento e acompanhamento para o estudo do JAV-RARAS. Inicialmente, mapeamos a jornada de cuidados nos centros participantes do estudo JAV-RARAS para 21 doenças raras, quantificando o tempo e os recursos associados às etapas de diagnóstico, tratamento e acompanhamento. Os custos diretos foram obtidos por meio de entrevistas e registros de dados administrativos de profissionais de enfermagem e médicos em centros de saúde nas regiões Sudeste e Nordeste do país. Foi conduzida juntamente a análise de custos diretos, identificação de atividades dentro da jornada do paciente com PAF, alocação de recursos, custo por unidade e mensuração do tempo de duração da atividade. Resultados: Em média, o custo anual para um paciente em tratamento de PAF é de R$198.960,40, sendo a maior parte alocado nas despesas com medicamentos. A divisão dos custos inclui recursos humanos (R$253,20), materiais (R$541,74), medicamentos (R$197.240,40) e exames (R$925,02). Todo o gasto anual é dedicado exclusivamente ao tratamento da doença. Em relação à origem dos recursos, aproximadamente 99% são provenientes de centros de tratamento, enquanto o 1% restante é financiado pelo Sistema Único de Saúde (SUS) em outras localidades. Conclusões: Este estudo destaca a importância de levantar e analisar custos e acesso a intervenções terapêuticas no tratamento da PAF. A maior parte das despesas anuais é atribuída a medicamentos, enfatizando a necessidade de uma compreensão mais abrangente dos fatores que influenciam esses custos. Explorar alternativas terapêuticas mais eficazes têm o potencial de reduzir custos a longo prazo e melhorar os resultados clínicos dos pacientes.

Abstract
Introduction: RARAS (National Network of Rare Diseases) is a study developed in reference hospitals for the management of patients with rare diseases in the Unified Health System (SUS). Objective: To quantify and describe the costs associated with the care journey of patients with Familial Amyloid Polyneuropathy (FAP) as part of the JAV-RARAS project, which is a component of the National Network of Rare Diseases (RARAS) in Brazil, applying Time-driven Activity-Based Costing (TDABC). Material and Method: The time-driven activity-based costing (TDABC) study consists of data collected from diagnosis, treatment, and follow-up for the JAV-RARAS study. Initially, we mapped the care journey in the centers participating in the JAV-RARAS study for 21 rare diseases, quantifying the time and resources associated with the stages of diagnosis, treatment, and follow-up. Direct costs were obtained through interviews and administrative data records of nursing professionals and physicians in health centers in the Southeast and Northeast regions of the country. The analysis of direct costs, identification of activities within the patient's journey with FAP, allocation of resources, cost per unit and measurement of the duration of the activity were conducted together. Results: On average, the annual cost for a patient undergoing FAP treatment is R$198,960.40, with the majority allocated to medication expenses. The division of costs includes human resources (R$253.20), materials (R$541.74), medication (R$197,240.40) and exams (R$925.02). All annual expenditure is dedicated exclusively to the treatment of the disease. Regarding the origin of the resources, approximately 99% come from treatment centers, while the remaining 1% is financed by the Unified Health System (SUS) in other locations. Conclusions: This study highlights the importance of assessing and analyzing costs and access to therapeutic interventions in the treatment of FAP. The majority of annual expenditures are attributed to medications, highlighting the need for a more comprehensive understanding of the factors that influence these costs. Exploring more effective therapeutic alternatives has the potential to reduce long-term costs and improve patient outcomes.
