Resumo
A miltefosina é uma nova alternativa terapêutica de uso oral disponibilizada recentemente no Brasil para o tratamento Leishmaniose Tegumentar (LT). A partir de 2021, o medicamento foi disponibilizado e os estados iniciaram a sua implementação. Objetivo: Este trabalho apresenta como objetivo descrever o Perfil dos pacientes em uso da miltefosina e indicadores de acesso ao medicamento no estado de Minas Gerais no período de 2021 a 2023. Material e Método: Análise retrospectiva de documentos administrativos da dispensação de miltefosina em Minas Gerais no período de 06/2021 a 11/2023. Foram examinadas instruções, notas orientativas e pedidos de miltefosina para o tratamento da LT. Os dados foram compilados cruzando informações sobre a solicitação do medicamento e a resposta de autorização ou recusa emitido pelo Centro de Referência em Leishmaniose (CRL-IRR). A análise de dados abordou variáveis relacionadas ao Perfil sociodemográfco e clínico dos usuários, além de características sobre o acesso ao medicamento, processo de solicitação, prescrição e dispensação. Utilizando estatística descritiva, foram caracterizadas as solicitações do medicamento. O projeto foi aprovado pelo Comitê de Ética em Pesquisa (CEP) do IRR (CAAE: 45481421.4.0000.5091) e parceria frmada com Secretaria de Estado da Saúde de Minas Gerais (SES-MG) (processo nº 1320.01.0138554). Resultados: Nos 30 meses seguintes à disponibilização da miltefosina, foram identifcadas 260 solicitações, sendo 6 indeferidas, 13 duplicadas (registros inválidos ou não concluídos) e 5 que não iniciaram o tratamento, resultando em 236 pacientes iniciaram o tratamento. No Perfil dos pacientes que usaram a miltefosina predomina o sexo masculino (66,1%), com mediana de idade de 63 anos, autodeclarados pardos (40,7%), com leishmaniose cutânea (78,4%) e casos novos da doença (58,4%). Dentre as 79 mulheres em uso da miltefosina, destaca-se que apenas 15% estavam em idade fértil. O tempo médio de análise das solicitações foi de 4 dias (3,7±10,7) e tempo médio até a dispensação do medicamento foi de 33 dias (32,8±16,4), sendo a região centro a que apresentou menor tempo, 4 dias (3,7±10,7), e as regiões leste do sul e noroeste apresentaram atrasos signifcativos, 55±84 e 53±70 dias respectivamente. Pacientes atendidos nas regiões Centro (191,9±169,4) e Norte (192,6±198,8) percorreram as maiores distâncias entre o local de residência e o serviço de saúde. Conclusão: Os resultados do projeto revelam dados importantes sobre o acesso ao primeiro medicamento oral para tratar a leishmaniose tegumentar em Minas Gerais. Foram observadas disparidades em diferentes regiões do estado, as regiões Centro e Norte concentram o maior número de notifcações de LT. As diferenças entre as regiões sugerem a necessidade de monitorar o acesso e identifcar barreiras e facilitadores para a implementação da miltefosina em todo o estado.

Abstract
Miltefosine is a new oral therapeutic alternative recently made available in Brazil for the treatment of Cutaneous Leishmaniasis (TL). The drug has been available since 2021 and the states have begun to implement it. Objective: This study aims to describe the profile of patients using miltefosine and indicators of access to the drug in the state of Minas Gerais from 2021 to 2023. Material and Method: Retrospective analysis of administrative documents for the dispensing of miltefosine in Minas Gerais from 06/2021 to 11/2023. Instructions, guidance notes and requests for miltefosine for the treatment of TL were examined. The data were compiled by cross-referencing information on the request for the drug and the authorization or refusal response issued by the Leishmaniasis Reference Center (CRL-IRR). Data analysis addressed variables related to the sociodemographic and clinical profile of users, in addition to characteristics regarding access to the medication, request process, prescription and dispensing. Using descriptive statistics, the requests for the medication were characterized. The project was approved by the Research Ethics Committee (CEP) of the IRR (CAAE: 45481421.4.0000.5091) and a partnership was established with the State Department of Health of Minas Gerais (SES-MG) (process number 1320.01.0138554). Results: In the 30 months following the availability of miltefosine, 260 requests were identified, of which 6 were denied, 13 were duplicated (invalid or incomplete records) and 5 did not start treatment, resulting in 236 patients who started treatment. The profile of patients who used miltefosine was predominantly male (66.1%), with a median age of 63 years, self-declared brown skin color (40.7%), with cutaneous leishmaniasis (78.4%) and new cases of the disease (58.4%). Among the 79 women using miltefosine, it is noteworthy that only 15% were of childbearing age. The average time to analyze the requests was 4 days (3.7±10.7) and the average time until the medication was dispensed was 33 days (32.8±16.4), with the central region having the shortest time, 4 days (3.7±10.7), and the eastern, southern and northwestern regions having significant delays, 55±84 and 53±70 days, respectively. Patients treated in the Central (191.9±169.4) and North (192.6±198.8) regions traveled the longest distances between their place of residence and the health service. Conclusion: The results of the project reveal important data on access to the first oral medication to treat cutaneous leishmaniasis in Minas Gerais. Disparities were observed in different regions of the state, with the Central and North regions concentrating the highest number of LT notifications. The differences between regions suggest the need to monitor access and identify barriers and facilitators for the implementation of miltefosine throughout the state.
