Abstract
Introduction: The technique of using insulin requires understanding a series of steps, and therefore, guidance on its use should be complemented by educational materials. In Brazil, a significant portion of the population is considered functionally illiterate, which reinforces the importance of developing educational materials in accessible language. Objective: In this context, this study aimed to compare the readability of an educational material on insulin use developed in simple language in relation to the patient information leaflet. Material and Method: The educational material was developed from the patient information leaflet (Novolin® N FlexPen® Patient Information Leaflet - Ministry of Health (CCDS V.14_V4.0)), considering the guidelines of the ICICT Plain Language Guide. Readability analysis was performed using the NILC-metrix software, focusing on lexical, syntactic, and textual simplicity aspects. The assessment of lexical complexity included the adapted Dale-Chall formula, the Fles-ch Index, and Honoré's statistic. Regarding syntactic complexity, the proportion of sentences that do not follow the subject-verb-object structure and the frequency of sentences in the analytical passive voice were considered. In addition, the proportion of simple words was evaluated. The readability of the simplified educational material was compared to the patient information leaflet. Results and Discussion: Most readability indices indicated lower textual complexity in the educational material compared to the patient information leaflet. The adapted Dale-Chall Formula indicated greater ease of reading in the simplified educational material, with values of 10.42 for the patient information leaflet and 9.32 for the simplified educational material. The Flesch Index reinforced this trend, showing a higher score in the educational material (77.10 vs. 51.71), suggesting greater ease of comprehension. Honoré's statistic also indicated lower textual complexity, with a lower value in the educational material (570.03 vs. 672.63), indicating clearer and more direct language for the reader. In the syntactic analysis, the educational material presented a higher proportion of sentences that do not follow the subject-verb-object structure (0 vs. 0.17) and greater use of the passive voice (0 vs. 0.0241), suggesting that lexical simplification was not accompanied by equivalent syntactic adjustments. Furthermore, there was an increase in the frequency of simple words (0.79 vs. 0.70). Conclusions: The results show that the simplified educational material presents better readability indices when compared to the patient information leaflet; however, it highlighted the need for syntactic adjustments to make it more accessible and easier to understand. It is important to emphasize that the evaluation of readability is only one step to ensure content accessibility, and testing with real users is essential.
