Introdução: O termo incongruência de gênero (IG) se refere a uma marcada dissidência entre o gênero vivenciado ou expresso e aquele que lhes foi atribuído no nascimento. Essa dissidência pode gerar sofrimento clinicamente significativo no indivíduo em diversas nuances da vida (1). Considerando as limitações dos exames clínicos (2) em mensurar os impactos sociais e emocionais da incongruência de gênero, tornase necessária a utilização de instrumentos disponíveis na literatura, entre os quais se destaca o Gender Preoccupation and Stability Questionnaire 2 (GPSQ2) (3). Objetivo: Determinar a estrutura fatorial da Gender Preoccupation and Stability Questionnaire 2 (GPSQ2) para a população transgênero em sua versão em portuguêsBR. Métodos: Foi realizado um estudo transversal em um ambulatório transexualizador durante o período de novembro de 2024 a abril de 2025. A amostragem foi realizada por conveniência, com os usuários que possuíam idade igual ou superior a 18 anos atendidos no ambulatório. A Análise Fatorial Confirmatória (AFC) foi realizada por meio do Software R (versão 4.4.3) e o pacote Lavaan (versão 0.619), conduzida seguindo a estrutura fatorial originalmente proposta composta por dois fatores: preocupação (compreendido pelos itens 1, 2, 4, 5, 7, 9, 12, 13) e estabilidade (composto pelos itens 3, 6, 8, 10, 11, 14). O método de estimação Diagonally Weighted Least Squares foi utilizado visando um melhor ajuste do modelo aos dados ordinais. A adequação da estrutura fatorial foi avaliada por meio da razão entre o Quiquadrado (χ2 ) e os graus de liberdade (gl) e por meio dos seguintes critérios de qualidade: Comparative Fit Index (CFI) (4), TuckerLewis Index (TLI) (5), Goodness of Fit Index (GFI), Root Mean Square Error of Approximation (RMSEA) e Root Mean Square of Residuals (SRMR). O presente estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Sergipe, sob o parecer: 79839224.7.1001.5546. Resultado e Conclusão: A amostra total foi constituída por 96 participantes, sendo 49 transgênero masculino, 42 transgênero feminino, 4 NãoBinários e um participante optou por não responder. A maioria dos participantes tinha até 35 anos, 39 tinham entre 18 e 24 anos e 43 tinham entre 25 a 34 anos. A maioria dos indivíduos tinha, no mínimo, ensino médio completo. De acordo com os indicadores de qualidade da AFC, o modelo bifatorial da GSPQ2 apresentou bom ajuste do modelo aos dados (CFI = 0,94; TLI = 0,93; RMSEA = 0,01; �²/df = 1,01). Os resultados indicam a validade da estrutura bifatorial originalmente proposta da GSPQ2 na avaliação da IG na população transgênero brasileira. Na sequência, fazse necessário analisar sua confiabilidade, incluindo consistência interna e estabilidade.

Abstract
Introduction: The term gender incongruence (GI) refers to a marked discrepancy between the experienced or expressed gender and the gender assigned at birth. This discrepancy may generate clinically significant distress in individuals across different aspects of life (1). Considering the limitations of clinical assessments (2) in measuring the social and emotional impacts of gender incongruence, the use of instruments available in the literature becomes necessary, among which the Gender Preoccupation and Stability Questionnaire 2 (GPSQ2) stands out (3). Objective: To determine the factorial structure of the Gender Preoccupation and Stability Questionnaire 2 (GPSQ2) for the transgender population in its Brazilian Portuguese version. Methods: A crosssectional study was conducted in a transgender healthcare outpatient clinic from November 2024 to April 2025. Convenience sampling was performed with users aged 18 years or older who were receiving care at the outpatient clinic. Confirmatory Factor Analysis (CFA) was performed using R software (version 4.4.3) and the Lavaan package (version 0.619), following the originally proposed factorial structure composed of two factors: preoccupation (including items 1, 2, 4, 5, 7, 9, 12, and 13) and stability (comprising items 3, 6, 8, 10, 11, and 14). The Diagonally Weighted Least Squares estimation method was used to achieve better model fit for ordinal data. The adequacy of the factorial structure was assessed through the ratio between the Chisquare (χ²) and degrees of freedom (df), as well as the following quality criteria: Comparative Fit Index (CFI) (4), TuckerLewis Index (TLI) (5), Goodness of Fit Index (GFI), Root Mean Square Error of Approximation (RMSEA), and Standardized Root Mean Square Residual (SRMR). This study was approved by the Research Ethics Committee of the Federal University of Sergipe, under approval number: 79839224.7.1001.5546. Results and Conclusion: The total sample consisted of 96 participants, including 49 transgender men, 42 transgender women, 4 nonbinary individuals, and one participant who chose not to respond. Most participants were up to 35 years old; 39 were between 18 and 24 years old and 43 were between 25 and 34 years old. Most individuals had at least completed high school education. According to the CFA quality indicators, the twofactor model of the GPSQ2 showed good model fit to the data (CFI = 0.94; TLI = 0.93; RMSEA = 0.01; χ²/df = 1.01). The results indicate the validity of the originally proposed twofactor structure of the GPSQ2 for assessing GI in the Brazilian transgender population. Subsequently, it is necessary to analyze its reliability, including internal consistency and stability.
