Resumo
Introdução: O eculizumabe é atualmente o único tratamento disponível para a Hemoglobinúria Paroxística Noturna (HPN). Esse medicamento foi incorporado pela CONITEC - Portaria Nº 77/2018 e listado no Componente Especializado da Assistência Farmacêutica, sendo necessária a elaboração do Protocolo Clínico e Diretrizes Terapêuticas (PCDT) para disponibilização do medicamento em até 180 dias. Nesse contexto, há o desafio de elaborar de forma rápida um PCDT robusto, baseado em evidências e com validação da sociedade, para uma doença rara e com grande impacto financeiro para o SUS. Método: O escopo do PCDT foi direcionado pela portaria de incorporação do eculizumabe, que definia a população beneficiada e os critérios de uso do medicamento. Foram elaboradas as perguntas de pesquisa e realizadas buscas sistemáticas na literatura. Para identificação dos principais especialistas em HPN e validação do escopo do PCDT, foi utilizado o Método “bola de neve”. Em seguida, foi realizado um painel para apresentação da minuta do PCDT com um grupo representado por especialistas, sociedades médicas e de pacientes, com a finalidade de validar o texto, esclarecer pontos críticos e finalizar a elaboração do PCDT. Resultado: Após a busca por evidências, foram estabelecidos os critérios de elegibilidade e de interrupção do tratamento. Através do Método bola de neve, cinco especialistas de referência em HPN foram contatados, representando todas as regiões do Brasil, e suas contribuições foram utilizadas para elaborar a versão preliminar do PCDT. O texto foi apresentado e discutido em um painel com 9 integrantes, para elaboração da versão final do PCDT, que foi apresentada à Conitec e aprovada pelo plenário. Na Consulta Pública, das 309 contribuições recebidas, 85% considerou o PCDT como muito bom ou bom. Conclusão: O PCDT da HPN foi elaborado e validado com agilidade, em aproximadamente 60 dias, tendo boa aceitação da sociedade.

Abstract
Introduction: Eculizumab is currently the only treatment available for paroxysmal nocturnal hemoglobinuria (PNH). It was recently incorporated by Conitec and listed in the specialized component of SUS, making it necessary to develop a Clinical Protocol and Therapeutic Guidelines (PCDT), so that the medicine can be distributed to the population within 180 days. In this context, the challenge arises of rapidly developing a robust, evidence-based and society-validated PCDT for a rare disease with major financial impact on SUS. Method: The scope of the PCDT was directed by the incorporation of eculizumab, which defined the beneficiary population and the criteria for its use. The research questions were elaborated and systematic searches were made in the literature. In order to identify the main HPN medical specialists and to validate the scope of the PCDT, the snowball Method was used. Then, a panel was held to present the draft of the PCDT with a group of experts, medical societies and patient representatives, to validate the text, clarify critical points and finalize the elaboration of the PCDT. Results: After searching for evidence, eligibility and treatment interruption criteria were established. Through the snowball Method, five PNH experts were contacted, representing all five regions of Brazil, and their contributions were used to prepare the draft version of the PCDT. The text was presented and discussed in a 9-member panel to define the final version of the PCDT, which was presented to Conitec and approved by the plenary. In the Public Consultation, of the 309 contributions received, 85% considered the PCDT to be very good or good. Conclusion: The HPN PCDT was elaborated and validated with agility, in approximately 60 days, with good acceptance by society
