Resumo
Introdução: O angioedema hereditário (AEH) por deficiência de C1-esterase é uma doença genética rara caracterizada pela deficiência ou disfunção do inibidor do C1-INH, também denominada tipo I ou tipo II. Na doença, o inibidor deficiente ou ausente deixa de atuar sob a calicreína plasmática, fatores de coagulação (XII e XI) e plasmina e desencadeiam elevação de bradicinina, vasodilatação, aumento da permeabilidade vascular, extravasamento de plasma e angioedema. Os episódios podem acometer diferentes partes do corpo, como o rosto, mãos, pés, trato gastrointestinal e vias aéreas superiores, po-dendo, nestes casos, ser potencialmente fatal. A abordagem terapêutica medicamentosa atual do AEH consiste no tratamento ou profilaxia, a curto ou longo prazo, das crises. O Sistema Único de Saúde re-comenda o uso regular de andrógenos atenuados e antifibrinolíticos. No entanto, essas duas farmacote-rapias apresentam, respectivamente, inconvenientes de segurança (reações adversas e contraindicação a gestantes e lactantes) e limitações de efetividade na literatura científica. Deste modo, esse o presente trabalho, se propôs a identificar no horizonte tecnológico medicamentos potenciais para a profilaxia de crises de AEH em pessoas com 2 anos ou mais de idade, gestantes ou lactantes, em alternativa aos andrógenos e antifibrinolíticos.Métodos: Foram realizadas buscas estruturadas nos campos de pesquisa das bases de dados Clinical-Trials.gov e Cortellis™, em julho de 2023, utilizando as seguintes estratégias: ClinicalTrials: Recruiting, Not yet recruiting, Active, not recruiting, Completed, Enrolling by invitation Studies | Interventional Studies | Angioedemas, Hereditary | Phase 2, 3, 4; e Cortellis: Current Development Status (Indication (Hereditary angioedema) Status (Launched or Registered or Pre-registration or Phase 3 Clinical or Phase 2 Clinical). Foram considerados estudos clínicos de fases 2, 3 ou 4 inscritos no ClinicalTrials; e tecnolo-gias com registro para a indicação clínica nos últimos 5 (cinco) anos na Agência Nacional de Vigilância Sanitária (Anvisa), European Medicines Agency (EMA) ou U.S. Food and Drug Administration (FDA).Resultados: Foram identificadas 7 tecnologias potenciais para o tratamento do AEH para maiores de dois anos, gestantes ou lactantes, entre as fases 2 e 4 de desenvolvimento. Uma tecnologia atendeu integralmente à idade da população, o garadacimabe (anticorpo anti-FXIIa, de uso subcutâneo (SC)). Outras 4 tecnologias atenderam parcialmente à idade da população avaliada: 2 inibidores de pré-cali-creína (donidalorsen e STAR-0215, de uso subcutâneo) e duas terapias gênicas (NTLA-2002 e BMN-331, de uso endovenoso). Além dessas, há 2 tecnologias já registradas na Anvisa, EMA e FDA que atendem integral ou parcialmente ao critério de idade, mas apresentam restrição à gestação e lactação: cloridrato de berotrastalte (de uso oral, para maiores de 12 anos) e lanadelumabe (SC, para maiores de 2 anos de idade).Discussão e conclusões: Há sete tecnologias no horizonte com potencial de proporcionar comodidade posológica, maior adesão, mais efetividade e menos reações adversas no tratamento profilático do AEH por deficiência de C1-esterase a pessoas com 2 anos de idade ou mais, gestantes ou lactantes.

Abstract
Introduction: Hereditary angioedema (HAE) due to C1-esterase deficiency is a rare genetic disease characterized by deficiency or dysfunction of the C1-INH inhibitor, also called type I or type II. In this disease, the deficient or absent inhibitor fails to act on plasma kallikrein, coagulation factors (XII and XI) and plasmin, triggering elevated bradykinin, vasodilation, increased vascular permeability, plasma leakage and angioedema. Episodes can affect different parts of the body, such as the face, hands, feet, gastrointestinal tract and upper airways, and in these cases can be potentially fatal. The current drug therapeutic approach to HAE consists of short- or long-term treatment or prophylaxis of crises. The Unified Health System recommends the regular use of attenuated androgens and antifibrinolytics. However, these two pharmacotherapies present, respectively, safety drawbacks (adverse reactions and contraindications for pregnant and lactating women) and limitations of effectiveness in the scientific literature. Thus, this study aimed to identify, on the technological horizon, potential drugs for the prophylaxis of HAE attacks in people aged 2 years or older, pregnant or lactating, as an alternative to androgens and antifibrinolytics. Methods: Structured searches were carried out in the search fields of the Clinical-Trials.gov and Cortellis™ databases, in July 2023, using the following strategies: ClinicalTrials: Recruiting, Not yet recruiting, Active, not recruiting, Completed, Enrolling by invitation Studies | Interventional Studies | Angioedemas, Hereditary | Phase 2, 3, 4; and Cortellis: Current Development Status (Indication (Hereditary angioedema) Status (Launched or Registered or Pre-registration or Phase 3 Clinical or Phase 2 Clinical). Clinical studies of phases 2, 3 or 4 registered in ClinicalTrials were considered; and technologies with registration for the clinical indication in the last 5 (five) years in the National Health Surveillance Agency (Anvisa), European Medicines Agency (EMA) or US Food and Drug Administration (FDA). Results: Seven potential technologies for the treatment of HAE for pregnant or lactating women over two years of age were identified, between phases 2 and 4 of development. One technology fully met the age of the population, garadacimab (anti-FXIIa antibody, for subcutaneous (SC) use). Another 4 technologies partially met the age of the population evaluated: 2 pre-kallikrein inhibitors (donidalorsen and STAR-0215, for subcutaneous use) and two therapies (NTLA-2002 and BMN-331, for intravenous use). In addition to these, there are 2 technologies already registered with Anvisa, EMA and FDA that fully or partially meet the age criterion, but have restrictions on pregnancy and lactation: berotrastalte hydrochloride (for oral use, for those over 12 years of age) and lanadelumab (SC, for those over 2 years of age). Discussion and conclusions: There are seven technologies on the horizon with the potential to provide dosage convenience, greater adherence, greater effectiveness and fewer adverse reactions in the prophylactic treatment of HAE due to C1-esterase deficiency in people 2 years of age or older, pregnant or lactating.
