Resumo
Introdução: O relato seletivo de desfechos prejudica o conhecimento sobre os efeitos das tecnologias em saúde, em especial nas revisões sistemáticas. A falta de transparência no relato pode impactar negativamente a confiança dos pacientes, médicos e profissionais de saúde na evidência científica dis-ponível.Métodos: A partir de revisão sistemática sobre efeitos adversos renais dos anti-inflamatórios inibidores seletivos da cicloxigenase-2 (protocolo registrado no PROSPERO: CRD42022380227), o relato seletivo de desfecho foi avaliado nos artigos incluídos por meio do item 6 da ferramenta Cochrane Risk of Bias. Classificamos como alto risco de viés nesse item os estudos que (1) adotaram limiar de incidência de eventos (mínimo de ocorrência do evento para relatar reações adversas) ou (2) informaram somente eventos adversos pré-especificados ou previamente definidos, sem relatar todos os eventos adversos que ocorreram ou (3) não reportaram eventos altamente suspeitos (ex.: relatada interrupção do tra-tamento por determinada reação, mas não houve relato das reações especificamente). Calculamos a frequência de estudos com relato seletivo e os eventos renais ausentes em estudos com relato seletivo foram considerados eventos adversos renais não relatados.Resultados: 50 estudos incluídos, (reportados em 46 artigos) foram incluídos na revisão e analisados no presente estudo. A maior parte dos estudos (42/50) foi financiada pela indústria farmacêutica. O relato seletivo de reações adversas foi observado em 31 estudos, 19 estudos adotaram limiar de incidência para relatar reações adversas: 3% (n=9), 5% (n=7), 2% (n=2), 1% (n=1) e 12 estudos reportaram exclusivamente eventos adversos pré-especificados. Os seguintes eventos potencialmente não foram relatados nos estudos: creatinina (n=28), desordens renais ou urinárias (n=31), glicosúria (n=30), hematúria (n=30), infecção urinária (n=28), retenção urinária (n=26), ureia nitrogenada (n=29), eventos renais ou hipertensivos (n=31) e descontinuação devido a creatinina elevada (n=30).Discussões e conclusões: Relato seletivo foi identificado em ensaios clínicos randomizados que in-vestigaram os efeitos adversos renais dos inibidores seletivos da cicloxigenase-2. O financiamento da indústria farmacêutica possivelmente influenciou no relato seletivo. O relato transparente dos estudos, também nas reações adversas, é necessário para assegurar resultados confiáveis e a segurança dos pacientes. A ausência dessas informações potencialmente subestima riscos que esses medicamentos podem causar em doses terapêuticas.

Abstract
Introduction: Selective reporting of outcomes undermines knowledge about the effects of health technologies, especially in systematic reviews. Lack of transparency in reporting can negatively impact the confidence of patients, physicians, and health professionals in the available scientific evidence. Methods: Based on a systematic review on renal adverse effects of selective cyclooxygenase-2 inhibitors (PROSPERO registered protocol: CRD42022380227), selective outcome reporting was assessed in the included articles using item 6 of the Cochrane Risk of Bias tool. We classified as high risk of bias in this item the studies that (1) adopted an event incidence threshold (minimum occurrence of the event to report adverse reactions) or (2) reported only pre-specified or previously defined adverse events, without reporting all adverse events that occurred or (3) did not report highly suspected events (e.g., reported treatment discontinuation due to a certain reaction, but there was no report of the reactions specifically). We calculated the frequency of studies with selective reporting and the renal events absent in studies with selective reporting were considered as unreported renal adverse events. Results: 50 included studies (reported in 46 articles) were included in the review and analyzed in the present study. Most of the studies (42/50) were funded by the pharmaceutical industry. Selective reporting of adverse reactions was observed in 31 studies, 19 studies adopted incidence threshold to report adverse reactions: 3% (n=9), 5% (n=7), 2% (n=2), 1% (n=1) and 12 studies reported exclusively pre-specified adverse events. The following events were potentially not reported in the studies: creatinine (n=28), renal or urinary disorders (n=31), glycosuria (n=30), hematuria (n=30), urinary infection (n=28), urinary retention (n=26), urea nitrogen (n=29), renal or hypertensive events (n=31) and discontinuation due to elevated creatinine (n=30).Discussions and conclusions: Selective reporting was identified in randomized clinical trials investigating renal adverse effects of selective cyclooxygenase-2 inhibitors. Funding from the pharmaceutical industry possibly influenced selective reporting. Transparent reporting of studies, including adverse reactions, is necessary to ensure reliable results and patient safety. The lack of this information potentially underestimates the risks that these drugs may cause at therapeutic doses.
