Resumo
O câncer de mama é o tumor maligno mais frequente entre as mulheres do Brasil. Com o advento e disponibilização em larga escala da mamografia, aumentou consideravelmente o número de pacientes diagnosticadas em estádio iniciais, com maior chance de curabilidade. O tratamento, neste caso, envolve cirurgia que, na grande maioria das vezes, é conservadora, seguida ou não de tratamento sistêmico (quimio ou hormonioterapia) e radioterapia sobre toda a mama. Como observou-se que a maioria das recidivas locais ocorre no quadrante originário da neoplasia, pressupõe-se que a irradiação exclusiva deste quadrante, com potenciais ganhos em efeitos secundários (além de otimização do tempo de tratamento), atingiria os mesmos resultados de controle local. Foi feita, então, revisão sistemática da literatura com inclusão de 1 estudo randomizado que comparou a radioterapia intra-operatória com elétrons (IOeRT) ao tratamento padrão, radioterapia conformacional (RT 3D) no câncer de mama inicial. Observou-se pequeno aumento do número de falhas locais em 5 anos (4,4 vs. 0,4%), sem alteração, porém, na sobrevida global, dado o resgate efetivo com mastectomia. Quando se restringiu o estudo às mulheres com tumores de até 2,0 cm de diâmetro, graus I ou II, com axila negativa ou com até 3 linfonodos comprometidos e com tumores não classificados como triplo-negativos, chegou-se a um nível comparável de controle local (1,5% de recidivas em 5 anos), sem diferenças significativas no tocante aos efeitos secundários ao tratamento. Em análise econômica posterior, optou-se, então, pelo estudo de minimização de custos, sob a perspectiva das Operadoras de Saúde do sistema de saúde privado brasileiro, tendo como base os custos dos procedimentos descritos na Classificação Brasileira Hierarquizada de Procedimentos Médicos (CBHPM), 5ª edição. Nesta análise, observou-se que a aplicação da IOeRT implica em uma economia de recursos da ordem de 51,5% por tratamento, o que faz da IOeRT opção que leva a economia de recursos, quando utilizada no tratamento de mulheres com câncer de mama inicial, quando observados os critérios de inclusão descritos previamente.

Abstract
Breast cancer is the most common malignant tumor among women in Brazil. With the advent and widespread availability of mammography, the number of patients diagnosed in the early stages, with a greater chance of cure, has increased considerably. Treatment, in this case, involves surgery, which is mostly conservative, followed or not by systemic treatment (chemo or hormone therapy) and radiotherapy to the entire breast. Since it was observed that most local recurrences occur in the quadrant where the neoplasia originated, it is assumed that irradiation exclusively in this quadrant, with potential gains in side effects (in addition to optimizing treatment time), would achieve the same local control results. A systematic review of the literature was therefore carried out, including 1 randomized study that compared intraoperative electron radiotherapy ( IOeRT ) with the standard treatment, conformal radiotherapy (3D RT) in early breast cancer. A small increase in the number of local failures was observed at 5 years (4.4 vs. 0.4%), but there was no change in overall survival, given the effective rescue with mastectomy. When the study was restricted to women with tumors up to 2.0 cm in diameter, grades I or II, with negative axilla or with up to 3 lymph nodes involved and with tumors not classified as triple-negative, a comparable level of local control was achieved (1.5% of recurrences at 5 years), with no significant differences regarding side effects of treatment. In a subsequent economic analysis, a cost minimization study was then chosen, from the perspective of the Health Insurance Companies of the Brazilian private health system, based on the costs of the procedures described in the Brazilian Hierarchical Classification of Medical Procedures (CBHPM), 5th edition. In this analysis, it was observed that the application of IOeRT implies a saving of resources of around 51.5% per treatment, which makes IOeRT an option that leads to saving resources, when used in the treatment of women with early breast cancer, when the inclusion criteria described previously are observed.
