Resumo
O conhecimento da MPS tipo II na América Latina continua escasso, assim como o diagnóstico e o tratamento na região ainda são um grande desafio, junto com o elevado peso familiar que acarreta. No Brasil, a MPS tipo II é o tipo clínico mais comum e a elevada experiência e atenção do público sobre o assunto tornam o Brasil uma referência informal na região. Recentemente, o Ministro da Saúde do Brasil incorporou o tratamento no sistema de reembolso, e estão a ser desenvolvidos novos centros de tratamento multidisciplinares dos pacientes com a doença, enquanto nos demais países da região tal não sucede. O aumento dos ensaios clínicos na América Latina pode contornar o acesso aos tratamentos para MPS tipo II. Diversas ações podem ser realizadas para promover uma mudança significativa tanto nos pacientes quanto nos familiares que vivem com MPS tipo II. A conscientização sobre a doença para alunos de medicina e pediatras ou geneticistas pode moldar o futuro da MPS tipo II, em termos de diagnóstico precoce e correto para facilitar o acesso ao tratamento. O envolvimento das associações de pacientes e da sociedade pode levar a uma atenção correta à doença e criar um ambiente de pressão política para acelerar as melhorias em termos de acesso ao tratamento ou mesmo a aprovação de terapias inovadoras de forma rápida. Mudanças políticas precisam ser realizadas para promover o diagnóstico neonatal e criar um cenário favorável para o acesso ao tratamento por meio da alteração dos esquemas de reembolso ou pagamento.

Abstract
MPS type II awareness in Latin America remains weak and the diagnosis and treatment in the region are still a major challenge, together with high family burden. In Brazil, MPS type II is the most clinical type and the large experience as well as large public engagement on the subject become Brazil as an informal reference in the region. Recently, Brazilian Minister of Health has incorporated the treatment in the reimbursement system, and it is being developed new centers for multidisciplinary treatment of the patients with the syndrome, while in the remaining countries of the region that is not the case. Increasing clinical trials in Latin America can circumvent the access to the treatments for MPS type II. Several actions can be undertaken to promote a significant change in both, patients and families, living with MPS type II. Disease awareness to students in medical schools and to pediatricians or geneticists can shape the future of the MPS type II, in terms of earlier and correct diagnosis to fasten the access to treatment. Involvement of the patients’ associations and society may lead to correct attention to the disease and create an environment of political pressure to accelerate improvements in terms of treatment access or even approval of breakthrough therapies in a fast way. Political changes need to be undertaken to promote neonatal diagnosis and create a favorable landscape for treatment access by alteration of reimbursement or payment schemes
